Attendance & No-Show Acknowledgment Form

This form is used to document attendance violations, including arriving more than 15 minutes late for a scheduled shift and failure to report for a scheduled shift (No-Show).

Team members who arrive more than 15 minutes late are required to complete this form.

A No-Show is considered a serious attendance violation and requires completion of this form. Two No-Show incidents may result in termination of employment.

Please complete this form accurately and honestly. You may provide any circumstances you believe management should consider when reviewing the incident.
